Your 2019 1099-R tax form was mailed to the address we have on file in our system, which differs from the address provided to us by the U.S. Postal Service. The full first-class postage rate was used so that, if at all possible, the post office will forward your 1099-R to your current address.
For your security, we can only update your address in our system when you inform us in writing with a completed and signed
Change of Address form. Once you have completed and signed the Change of Address form, please return it to our main office as soon as possible (our address is below). We cannot accept address changes via phone, fax or email.
If you do not receive your 1099-R by Friday, February 14, or if you need a Change of Address form mailed to you, please call our main office at 617-679-6877 to request a copy.
