At the end of January 2021, we will be mailing you your 1099-R tax form and Benefit Verification form together in the same envelope because you have received a payment of either a retirement or survivor benefit from the MTRS in 2020. To avoid any delay in receipt of these important forms, please make sure we have your current mailing address.
If you have recently moved—or will be moving to a new or temporary address in January 2021—please let us know BEFORE December 31, 2020 to avoid delays in receiving your 1099-R and Benefit Verification form.
You can update your address by completing a
Change of Address form, available on our website. For your security, we cannot accept change of address requests via email, fax or over the phone.
