At the end of January 2022, we will be mailing you your 1099-R tax form because you have received a payment of either a retirement or survivor benefit from the MTRS in 2021. To avoid any delay in receipt of these important forms, please make sure we have your current mailing address.
If you have recently moved—or will be moving to a new or temporary address in January 2022—please let us know in writing with a Change of Address form BEFORE December 10, 2021 to avoid delays in receiving your 1099-R form.
You can update your address by completing a
Change of Address form, available on our website. For your security, we cannot accept change of address requests via email, fax or over the phone.
